“Attention potential class members in Vodak v. City of Chicago:

In June of 2012, the Chicago City Council approved the $6.2 million settlementin Vodak v. City of Chicago, the class action brought on behalf of individuals who were detained and/or arrested on Chicago Avenue, east of Michigan Avenue, on March 20, 2003.

On July 23, 2012, Class Action Administration, Inc., the claims administrator, mailed written notices and claim forms to class and potential class members.

Class members must mail their completed claim form to Class Action Administration, Inc. in enough time so that it is received by October 22, 2012 if they want to receive their share of the settlement proceeds, or to take any other action with respect to the settlement………………..”
